We invite you to enthusiastically approach your personal practice. However, we encourage you to use care when determining your own ability to do the exercises offered in our sessions. Do not aggravate an existing injury. If you have a doubt as to whether yoga would be recommended if you have a particular injury or condition, please consult your health care professional. The instruction and advice presented in our sessions, and/ or in any class you may attend that is taught by Urban Yoga Chicago and its representatives is not meant to be used in lieu of professional medical or psychological care for the treatment of any condition. There are certain limitations to the practice of yoga for pregnant and menstruating women. Please exercise personal responsibility and ask questions until you understand the answers.
In checking the box below I agree to the following:
That I am participating in yoga classes and workshops offered by Urban Yoga Chicago during which I will receive information about yoga and wellness. I recognize that yoga involves physical movements as well as an opportunity for relaxation, re-education and relief of muscular tension. As is the case with any physical activity, the risk of injury, even serious or disabling, is always present and cannot be entirely eliminated. I am fully aware of the risks and hazards involved. If I experience any pain or discomfort, I will listen to my body, adjust the posture and ask for support from the teacher. I will breathe smoothly.
I understand that it is my responsibility to consult with a physician prior to and regarding my participation in yoga classes and workshops and that yoga is no substitute for medical attention, examination, diagnosis and treatment. I represent and warrant that I am physically fit and I have no medical condition which would prevent my full participation in yoga classes and workshops offered by Urban Yoga Chicago.
In consideration of being permitted to participate in yoga class or workshop, I agree to assume full responsibility for any risks, injuries or damages, known or unknown, which I might incur as a result of participating in the program. I understand that yoga practice and/or specific poses are not recommended for individuals with certain conditions (i.e. cardiac illness, pregnancy, post-surgery). I affirm that I alone am responsible to decide whether to practice yoga and therefore assume the risk of yoga practice and release Urban Yoga Chicago and the teacher(s) employed or subcontracted by Urban Yoga Chicago from any liability claims.
